PAPERWORK DEADLINE
All paperwork must be turned in by August 10, 2025. We cannot guarantee your paperwork will be processed by the first practice if it is submitted after this date.
MALONE UNIVERSITY ATHLETIC PARTICIPATION REQUIREMENT
The Malone Sports Medicine office utilizes electronic medical records through the Sportsware records software. No paper copies of electronic forms will be accepted. In order to complete your pre-participation medical paperwork you must use your account in the Sportsware system.  Â

____ Medical Consent, Release, and Privacy Form (Found in the attachment tab in Sportsware)
____ Malone Secondary Athlete Insurance Letter (Found in the attachment tab in Sportsware)
____ Emergency Contacts (To be completed in the Emergency tab in Sportsware)
____ ImPACT Baseline Test (ONLY if you suffered a concussion last school year)
____ Copy of Insurance Cards (to be uploaded to the Insurance tab in Sportsware)
____ Sway Balance Assessment (ONLY if you suffered a concussion last school year) (complete when you report to campus)
Parent/Guardian signatures are required on ALL forms for student-athletes that are under 18 years of age.
