PAPERWORK DEADLINE
All paperwork must be turned in by August 10, 2025.  We cannot guarantee your paperwork will be processed by the first practice if it is submitted after this date.

MALONE UNIVERSITY ATHLETIC PARTICIPATION REQUIREMENT
The Malone Sports Medicine office utilizes electronic medical records through the Sportsware records software.  No paper copies of forms will be accepted.  In order to complete your pre-participation medical paperwork you must use your account in the Sportsware system.

____ Medical Consent, Release, and Privacy Form (Found in the Attachment tab in Sportsware)
____ Malone Secondary Athlete Insurance Letter (Found in the Attachment tab in Sportsware)
____ Emergency Contacts (Found in the Emergency tab in Sportsware)
____ ImPACT Baseline Test (mandatory for ALL sports) ( download here)
____ Copy of Insurance Cards 
____ Sickle Cell Test Results (More info on how to obtain results here ) *REQUIRED BY NCAA FOR ALL ATHLETES*
____ Sway Balance Assessment (mandatory ALL Sports) (completed with the sports medicine staff when you report to campus)
Parent/Guardian signatures are required on ALL forms for student-athletes that are under 18 years of age.
